Internal notification system

DH Dekor, spol. s.r.o. (hereinafter referred to as the “company”), in accordance with Act No. 171/2023 Coll., on the protection of whistleblowers (hereinafter referred to as the “law”), introduces and further regulates the rules of the internal reporting system. The purpose of these rules is to provide channels for reporting potential violations, to protect the identity of whistleblowers, and to protect them from retaliation. Illegal conduct in the sense of the Internal Notification System of DH Dekor, spol. s.r.o. means an illegal act that has occurred or is expected to occur at the company and which: has the characteristics of a crime has the characteristics of a misdemeanor for which the law stipulates a fine of over CZK 100,000 violates other legal regulations listed in § 2 paragraph 1 letter d) of the Act. Any notifications are received and assessed by the relevant persons and further proceed mainly in accordance with §11 and §12 of the Act. The submitted notification must contain information about the name, surname and date of birth, or other information from which it is possible to infer the identity of the notifier. The notification does not have to contain these data if it was submitted by a person whose identity is known to the relevant person. DH Dekor, spol. s.r.o. designated the following relevant persons: Jan Matějů, phone: + 420 565 504 201 Radek Mojžíš, phone: + 420 565 504 202 DH Dekor, spol. s.r.o. established an internal notification system for all notifications made under the Act and for this purpose establishes the following methods of notification: electronically to the e-mail address oznameni@dhd.cz in writing to the address of the employer DH Dekor, spol. s.r.o., Lnářská 1602, 393 01 Humpolec, must be visibly marked “DO NOT OPEN, to the hands of the relevant person” by phone to the indicated number of the relevant person, from Monday to Friday 6:00 a.m. – 2:00 p.m in person, if the whistleblower so requests In the case of a notification through channels other than those mentioned above, the Company cannot guarantee the protection of the identity of the notifier and the information provided in the notification in accordance with the law. In addition to reporting through the Internal Reporting System, the whistleblower may use the Ministry of Justice’s external reporting system. In the case of suspicion of criminal activity, a report must be made to the authorities responsible for criminal proceedings, i.e. to the Police of the Czech Republic. The processing of personal data in connection with the submission of notifications, including their storage, takes place in accordance with generally binding legal regulations for the protection of personal data. By submitting a notification, the notifier agrees to the processing of his personal data. If it is proven that a knowingly false report was submitted, everything will be forwarded to the Ministry of Justice and the reporter will be fined up to CZK 50,000.
